Most medical billing companies say roughly the same things in their sales pitch. The differences that actually matter show up in how they operate day to day, not in their marketing copy.
Ask About Their Pricing Model, Not Just Their Rate
Percentage-based pricing typically runs 4–10% of collections. The more important question is what the percentage is calculated on: gross charges or net collections — this can matter more than the headline rate.
Ask How They Handle Your Specific Specialty
Coding and payer rules for behavioral health, pain management, dermatology, and primary care have almost nothing in common. Ask specifically who on their team has experience with your specialty.
Ask What Happens When a Claim Gets Denied
Ask for their actual denial rate and clean claim rate, and whether they have a defined denial process with root-cause tracking and appeal deadlines.
Ask About Reporting, and Ask to See a Sample
You should get regular reporting on collections, AR aging, denial trends, and clean claim rate — ask to see a sample report before signing.
In-House vs. Outsourced: When Each Makes Sense
In-house billing can work for larger practices with volume to justify a well-trained team. For most independent and small-to-mid practices, outsourcing is usually easier to justify than building that expertise internally.
